UNIT DESCRIPTION:

Candidate is available to provide services primarly to the nursing units and to support a rounding psychiatrist if indicated

JOB DESCRIPTION:

Provides independent and advanced professional Behavioral Health/ psychiatric care through evaluation, diagnostic impression, treatment planning, brief intervention, and family therapy as necessary; and providing linkage for community follow up.

Completes risk assessment on those patients that have attempted or indicated suicidality, using sound judgment and clinical decision making, in consultation with on call psychiatry, predicts degree of lethality and provides at risk patients linkage to appropriate level of treatment.

Works collaboratively with other disciplines, nursing units, and community providers to develop and apply care plans for the more complex or recurrent visit patient

Provides in house BEHAVIORAL HEALTH SERVICES, including differential diagnosis, consultation and treatment matching for substance abuse, consultation of acute psychiatric presentations that may be interfering with the patient’s ability to participate or receive medical care.

Provides formalized education to EGSMC (as assigned) to assure and maintain staff competency and safety while providing care for psychiatric patients awaiting medical clearance.

Reports directly to the Manager of Behavioral Health Services at EGSMC

MINIMUM QUALIFICATIONS:

· Master’s level or above in nursing, social work, psychology or other human services field.

· Graduate of an accredited school with an MA, MS, MSW, Ph.D ,Psy.D ,or Ed.D degree

· A minimum of 10 years experience in conducting consultations with patients and families to provide a
diagnostic impression, make appropriate treatment recommendations, and provide brief intervention
or community linkage

· Current professional license to practice in Colorado, as a PsyD, PH.D, ED.D, RN, L.A.C, LMFT,
LPC, LCSW

· Certified Addiction Counselor l,ll, or lll (preferred)

Current BLS certification

UNIT DESCRIPTION:

ART specialists conduct behavioral health assessments in the ELMC ED and at West Pines. In collaboration with a physician, the clinician determines level of care and facilitates admission or discharge. If a patient is discharged, clinician provides linkage to community services. Experience in an Emergency Department or inpatient psychiatric unit is essential. Fast paced environment which requires one to multi-task and make independent decisions.

JOB DESCRIPTION:

As a Master’s level or above licensed independent professional (aka, a physician extender), serving as a hospital approved credentialed consultant for the referring physician performs comprehensive psychiatric and chemical dependency evaluations for patients at ELMC, ESJH, and EWP. Thoroughly completes the following clinical evaluation forms as applicable: Exempla Behavioral Health Assessment/Reassessment Form; the Clinical Institute Withdrawal Assessment, Revised-Alcohol (CIWA-Ar); the Clinical Opiate Withdrawal Scale (COWS); the Alcohol and Drug Use Screening Tool/CAGE-Aid Questionnaire; the Benzodiazepine Withdrawal Symptom Questionnaire; the Folstein Mini Mental Status Examination; the Clinical Sobriety Checklist (CSC); the Suicide Risk Assessment Job Aid; the Emergency Mental Illness Report and Application (M-1); the Rights of Patients (M-2); the Psychiatric/Chemical Dependency Discharge Instructions; the Application for Emergency Commitment of Intoxicated or Incapacitated Person by Alcohol; the Application for Emergency Commitment of a Person Under the Influence of or Incapacitated by Drugs; Detox/Jail Release; Arapahoe House Information Sheet; Mental Health Crisis Call Report; the Patient Transfer Form; and/or the Direct Admission Form. Obtains collateral information when necessary, formulates provisional diagnoses (for clinical disorders or other conditions that may be a focus of clinical attention, personality disorders and mental retardation, psychosocial and environmental problems and global assessment of functioning) using the American Psychiatric Association’s Diagnostic and Statistical Manual. Makes treatment plan recommendations that may include, but are not limited to, admitting the patient, transferring the patient or discharging the patient. If the patient is not admitted, does not just make a simple referral, but instead, links the patient with the most appropriate provider or program based on the professional assessment that is done in conjunction with the patient, the family and the physician. Works closely with the patient, other ART team members, EWP physicians, EWP inpatient units, and emergency department and medical floor attending physicians, nurses and others in coordinating and communicating all aspects of patient care and disposition. Ensures/coordinates the business management of providing patient care services, including verifying patient demographics, identifying ability to pay for services, verifying insurance benefits, and pre-certifying services. Is knowledgeable of and adheres to EHC policies and procedures and all pertinent state and federal regulatory standards (e.g. JCAHO, HCFA, EMTALA, DBH, HIPAA).

MINIMUM QUALIFICATIONS:

· Master’s level or above in nursing, social work, psychology, counseling, or other human services field, with 3 years experience in psychiatric and chemical dependency assessments. License in a behavioral health field: LCSW, Licensed Psychologist, LPC, LFMT, RN or LAC.

· Graduate of an accredited school with an M.A., M.S., M.S.W., Ph.D., Psy.D. or Ed.D. degree.

· Knowledge and experience with managed care, inpatient medical and surgical care, emergency departments, as well as inpatient and outpatient behavioral health treatment.

PREFERRED QUALIFICATIONS:

1 year emergency assessment experience.

We are looking for an experienced clinician that is able to blend the 'old' and the 'new' in Addiction and Mental Health Care.

Evaluations and Assessments are a part of the work flow for our team members

JOB DESCRIPTION:

Job Duties:
Conducts Intakes for IOP Level of Care.
Develops and implements treatment plan for stabilization, mobilization and disposition appropriate for patient care within 72 hours of patient admission.
Conducts utilization and review to ensure clients receive authorization from payer source for treatment.
Facilitates psycho educational groups on addiction and recovery as part of core programming.
Participates in marketing and business development initiatives to insure viability of program.
Documents case management activities in patient’s medical records as required (including COMPUTER DOCUMENTATION)
Ensures quality of care for patient by maintaining clinical focus on the restoration of the patient to physical psychological safety.
Facilitates interdisciplinary case conferences detailing treatment plan and patient progress.
Consults and acts as liaison with all other members of the treatment team to mobilize patient resources insuring effective transition.
Provides educational information to patients and their families in individual and group settings.
Provides patient with information regarding community resources.
Provides aftercare planning and disposition.
Ensures compliance with applicable regulatory guidelines and requirements.
Develops community resources and educates other department members of those resources.
Participates in patient centered departmental meetings as well as non-departmental meetings and ongoing hospital committees as requested.
Maintains clinical skills through continuing education.
