Non-Benefited Status
This benefit allows RNs, Shift/Specialty Coordinators and Clinical Coordinators
to receive a pay differential of 20% for full-time status or 15% for part-time status
if the following benefits are waived: Medical, Dental, Vision, Health and Dependent
Care Reimbursement Accounts, Legal, Life and AD&D, Long Term Disability, Paid
Leave Time (PLT), Extended Ill Time (EIT), Tuition Reimbursement, and Bereavement
Leave.
Immediate Eligibility
Depending on your job status and when you are hired, you can be eligible almost
immediately for benefit coverage. New full-time and part-time employees are provided
benefits beginning the first of the month following your date of hire (provided
you enroll within 31 days of hire). Eligibility for the Exempla contributions to
the Retirement Plan begins January 1 following your hire date and is based on your
status - if you are full-time or part-time - and the number of eligible hours worked
during the year.
Free Medical & Dental Insurance
Exempla leads by example and continues its long tradition by offering free medical
and dental benefits to those eligible employees whose total household adjusted gross
income is less than $30,000 and for employees who have 20 or more years of service
with Exempla. Like we've said, we take care of our employees who take care of our
patients.
Medical
Exempla offers you two comprehensive HMO medical plans and a Point-of-Service (POS)
plan to choose from:
- Kaiser HMO
- CIGNA HMO
- CIGNA Open Access Plan POS
- CIGNA's prescription coverage for both the HMOs and POS is provided through SXC
Health Solutions
Dental
As an eligible Exempla employee, you can choose from the Delta Exclusive Provider
Organization (EPO) option or Delta Preferred PPO plan. Delta's EPO provides services
through a closed panel of dentists and the plan features no annual limit maximum
benefit and no deductibles. In addition, the Delta EPO also offers Orthodontia coverage
with no lifetime maximum (approximate 50% co-payment). The PPO option allows you
to see any dentist and the Delta participation level of the dentist will determine
the co-pay amount you will pay.
Vision
As an eligible Exempla employee, you can elect vision coverage through EyeMed ECPA
and select from a comprehensive list of eye care providers and services. The coverage
will provide one eye exam and one pair of lenses every 12 months. You can receive
a pair of frames every 24 months.
Life Insurance
As an eligible Exempla employee, you will receive your annual salary in Basic Life
Insurance coverage and an additional amount equal to your annual pay for accidental
death and dismemberment (AD&D) coverage all paid by Exempla. You will also have
the opportunity to pay for up to four times your annual pay in Supplemental Life
Insurance coverage that also includes AD&D coverage.
Dependent Life Insurance coverage plus AD&D is available to eligible Exempla
employees. You can elect to pay for spouse coverage at levels from $10,000 to $100,000
and for your child/children at levels from $5,000 to $15,000.
Long Term Disability Insurance
Exempla pays the premium for eligible employees for Long Term Disability Insurance
coverage in the event of you becoming disabled. This optional benefit can provide
for up to 67% of basic monthly earnings commencing after 90 days of disability.
To be eligible for payment you must submit a claim for approval.
Reimbursement Accounts
Reimbursement Accounts
Exempla provides you with the ability to reduce your overall tax burden through
popular health care and dependent care savings accounts.
- Healthcare Reimbursement Account - employee-contributed benefit dollars available
for reimbursement of medical expenses in a given benefit year up to $5,000.
- Dependent Care Reimbursement Account - employee-contributed benefit dollars available
for reimbursement of eligible child or elder day care expenses in a given benefit
year up to $5,000 ($2,500 if married and filing separate Federal tax return).
Legal Plan
The Hyatt Legal Plan provides you with comprehensive and affordable legal services
through one of the nation's largest providers of group legal services. Many employees
are signing up for group legal services for both the "peace-of-mind" of
having legal insurance as well as to enjoy many plan-paid services such as:
- Unlimited office and telephone advice and consultation
- Legal document preparation (mortgages, leases, etc.)
- Wills and trusts (including living wills and living trusts)
- Sale, purchase, or refinancing of your home
- Landlord negotiations
- Uncontested adoptions, guardianships and name changes
Retirement Plans
Retirement contributions by Exempla Healthcare have been increased. As an Exempla
employee, you can participate in our retirement plans, upon eligibility:
- 403 (b) Retirement Income Savings Plan allows employees to contribute up to the
maximum amount allowable by the IRS. The contributions are tax-deferred which decreases
your current taxable income. You can enroll to begin contributions after you have
received your first paycheck from Exempla.
- 401 (a) Exempla's Matching Contribution, for eligible full-time and part-time (20+
hours) status Exempla employees beginning January 1 following your date of hire.
Exempla will contribute $.50 for each dollar of the first 4% of pay that you contribute
(a maximum of 2% Exempla match).
- 401 (a) Exempla's Annual Basic Contribution, (with annual approval from the Exempla
Board of Directors) 3% - 6% of your eligible pay, depending on years of employment.
You are eligible if you have been employed for an entire calendar year, credited
with at least 1,000 eligible hours of service during the year, and are active on
December 31 of that year.
You will be vested (have ownership) in both of Exempla's contributions after 5 years
of vesting service (calendar years in which you are paid 1,000 or more eligible
hours).
Vacation, Holidays, and Paid Leave Time (PLT)
At Exempla, we understand the important balance of work and family by offering employees
flexible paid-leave-time accounts with accrual rates that increase as your years
of service grow.
Extended Illness Time (EIT)
At Exempla, we provide you with extended leave time to cover personal illness that
is four days or longer.
